The semi-annual reporting is due at the end of the month. Please mail your reports or e-mail them to me
as an attachment (yes...it can be done with what I am sending you this morning
:) ). My mailing address is: PPO Box 1477, Wildomar CA 92595-1477
DO NOT SEND THEM TO WING HQ. Be sure to submit a copy of the 34 to your squadron commander (Group Chaplains...one to your Group Commander as well).

Instructions for Form 34:
Reporting Period: type name or number of month first (i.e. 06-15-01; June 15, 2001) and it will self-correct. "From" is in cell C4; "To" is in cell
E4.
Charter Number: type number in cell A6
Name: type name in cell A9
Address: type address in cells A12,13,14
Check status: click on line 16 (A-E16). At the top of the page you will see the formula space. Place an "x" on the lines before the status (don't
worry about the line separating).
All the numbers placed in the "CAP Activities You Participated In" will total up themselves.
Line 46 "Total All Your Costs Listed Above" is only for the "Other Expenses Incurred", so don't worry if the number from CAP Activities and Other
Expenses don't add up.
To type the comments in the box beginning at line 82, first go to the formula bar at the top of the page and click in the blank area, then move
the cursor to the box. The same thing is true for the "Sound off" box at line 90.
Upon completion do a "Save As" - give it filename (i.e. lastname0601)
